How do I get a copy of my medical records?
Louisiana Orthopaedic Specialists has partnered with Sharecare to ensure the accurate and timely completion of your medical record requests.
Patients:
If you are a patient of ours and would like to request your medical or billing records, you can request them via MyChart. If you are requesting your records pre MyChart/EPIC implementation, you can email your request to medrec@laorthospec.com. You will be required to provide a valid email address and a government-issued ID.
For status updates of your request, please consent to receive status notifications about the progress of your request via email and text (SMS).
Only the patient, a parent/legal guardian, or the patient’s legal healthcare representative can sign the form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of an Advance Directive/Durable Power of Attorney for Healthcare/ Conservatorship.
Third Party Requesters:
If you are an attorney, insurance company, or any other entity requesting records from our facility, please click on the link below to upload your request along with the patient’s authorization.

DISABILITY/FMLA SUBMISSION FOR PATIENTS & REQUESTERS
How do I get a copy of medical records for Disability/FMLA?
Louisiana Orthopaedic Specialists has partnered with Sharecare for your FMLA and Disability form completion needs.
Authorization:
- Complete one Sharecare Request Completion Form for each of your employer’s forms. The Sharecare Form will be valid for 90 days unless otherwise specified on the form.
Cost per Form:
- Once you have submitted your form, Sharecare will contact you within 48 hours to collect payment for processing. The cost for them to process your paperwork is $35 per form.
Submit Your Paperwork for Completion:
- Submit your employer’s forms and the Sharecare Form.
